The Power of Kindness
The Home from Hospital service is for people who:
- Live alone
- Are isolated from their family/friends/local community
- Have caring responsibilities
- Where a carer is under pressure after the person they care for leaves hospital
They offer short term support with things such as:
- Weekly respite or companionship visits
- Help with shopping, light housework or light snack
- Managing day to day paper work
- Help with online access
- Confidence building
- Building links with community/making friends
- Support to access community/social activities
- Signposting to other sources of support
